Mesa Mental Health requires all providers on our panel to be recredentialed every two years. Our credentialing vendor, Hospital Services Corporation (HSC) will send a pre-filled application to all providers six months before the recredentialing due date. We are mandated by the National Committee for Quality Assurance (NCQA) to complete the recredentialing process within 6 months from the time of the provider’s signature on the application. Upon receipt of the application, HSC will verify all required elements and then provide Mesa Mental Health with the completed information. The Mesa Mental Health Credentials Committee will review all completed information prior to approving or denying the application.

A site visit will only be required at the time of recredentialing if the practitioner has moved since the previous credentialing or if the most recent site visit is older than five years.
