Independently licensed providers may join the Mesa Mental Health Provider Network by following these steps:

• Download and return the Pre-application, the Practitioner Profile and the IRS W-9 form.
• The pre-application will be reviewed by the Network Development Committee to determine the current needs within the network.
• If approved, you will be notified and sent a MMH contract and later a HSC Credentialing Application.
• If not approved, you may reapply in 12 months.
• A site visit will be necessary prior to credentialing
