To join the Mesa Mental Health Provider Network, just follow these steps:

• Download and return the Pre-application, the Practitioner Profile and the IRS W-9 form.
• The pre-application will be reviewed by the Network Development Committee to determine the current needs within the network.
• If approved you will be notified and sent a full application.
• If not approved you may reapply in twelve months.
• A site visit will be necessary prior to credentialing
